This form may be used to request temporary privileged access to enable an individual to perform actions that may affect computing systems, networking communication, or the accounts, files, data, or processes of other users. This form also enforces a strict elevated privilege rules or behavior and privacy and security agreement.

    • This form is intended to be used by Government of Guam Agency IT Personnel/Representatives. Download, complete, and submit this form to request to join a device(s) to the Government of Guam Domain, enable cache credentials, and/or request DHCP reservations. The purpose of AD credential caching is to allow access to the domain when access to the domain controller is unavailable – this process is usually only requested for mobile devices such as laptops and tablets for use outside the network. If cache credentials are requested, a single AD user account must be specified for that device.
